TO WHOM IT MAY CONCERN

As you know, through the years we have put together special mailings to help our clients with current issues. After the Terry Shiavo case, it occurred to me how many of us put off discussing life and death issues with our family members until it is too late.

My firm does not do any probate or estate planning but I wanted to do something for our clients to help them start this important discussion with family members. My firm, in conjunction with estate planner Dina Chernick are providing as a gift two forms:

1. A Statutory Advance Health Care Directive
2. An Estate Planning Questionnaire

You are welcome to photocopy these documents and give them to friends and family. You are also welcome to call my office with the name and address of anyone you would like me to send this information to.

If you need further information and want to consider doing an estate plan contact Dina Chernick at (310) 275-3520.

If you have any questions or comments on the documents Advanced Health Care Directive, or if you or someone you know has been injured in an accident, call me at (310) 288-0736.

Please consider the issues raised in the advance health care directive and make your wishes known to your medical providers and your family. Had Terry Shiavo done this it would have saved her family years of pain, indecision, animosity and enormous expense.

Sincerely,

Matison & Margolese, ALC
